PROPOSAL AGREEMENT

This contract is made between Jim Collins, (hereafter known as the Client) and Ponderosa Home Services (hereafter known as PHS). The Client will have the following services performed (described below), at the Client’s premises located at 13329 Davos Drive on Tuesday, October 6th, 2026.

Both parties agree as follows:

For services rendered, the client agrees to pay the proposed price of $600 for:

• Interior and exterior window washing

• Screen removal and cleaning

• Brushing and vacuuming screen tracks
